REACH, which stands for Remote Evaluation of Acute isCHemic stroke, is a turnkey telestroke application different from all other solutions
Using a hub and spoke model, neurologists can easily consult with patients from anywhere. The only requirements are a computer with webcam and a broadband internet connection. REACH makes it simple to intervene in those critical hours from home, hospital – even on vacation.
REACH is also ideally suited for use in the treatment of many other acute medical conditions, such as psychiatry, trauma, and burn care.
The consulting physician is typically affiliated with a hub hospital that can accept the patient if a transfer is required. The physician can conduct a consult from anywhere in the world, since the only hardware requirements are a computer with a broadband internet connection and a web- -connected video camera. No hardware is required at the hub.
Hospitals in rural or medically underserved areas (MUAs) serve as spokes in the REACH network. Spokes connect with the physician via a set of off-the-shelf components including a laptop, high resolution LCD monitor, keyboard, mouse and a high powered camera that allows the physician to perform detailed observations.
We assist spoke hospitals in the configuration and maintenance of their Emergency Room consult cart. In order to ensure maximum mobility and reliability, elements like a separate power source and wireless bridge are included. Once in place, it is simple to use: the ER staff simply accesses a web browser running on the cart’s laptop to register a patient and request a consult with the remote physician.
Designed by neurologists for neurologists, the REACH stroke consult is both easy and intuitive. All parties involved in the two-way consult are able to navigate through the relevant data, which is provided on a single screen. The secure, HIPAA-compliant web interface promotes efficient assessment, so recommendations can be made quickly.
For the spoke hospital, the web-based application presents:
- Patient registration screen

- Notes and recommendations entered by the consulting neurologist
For the neurologist, the web-based application presents:

To ensure streamlined communication, the spoke and consultant applications are synched in real-time, so that entries made at either end are visible to all parties immediately. Furthermore, all changes are recorded and time-stamped.
REACH securely stores all consult data and provides many documentation and reporting options, including both standardized and custom report templates. This capability simplifies HIPAA compliance, certification, and reimbursement for all parties.
As a telestroke network evolves, REACH provides reports that capture key performance statistics such as onset to treatment times, tPA decisions, and patient transfers. Available real-time and accessible with a secure connection to the REACH network web portal, these reports provide critical insights that help administrators gauge network performance and make strategic decisions to optimize their networks.
